Grocery Benefit Qualifying Conditions
Medical Mutual is excited to introduce a grocery benefit available with our MedMutual Advantage Secure HMO-POS plan offered in select counties. Members must have a qualifying chronic condition to receive a $50 monthly allowance to purchase healthy groceries.
Is the grocery benefit available on my plan?
In 2025, the grocery benefit will be available to MedMutual Advantage members with qualifying chronic conditions who are enrolled in our Secure HMO-POS plan (H6723-005-001) and reside in the following counties:
Ashland, Carroll, Columbiana, Cuyahoga, Geauga, Holmes, Lake, Lorain, Mahoning, Medina, Portage, Stark, Summit, Trumbull, Tuscarawas, Wayne, Brown, Butler, Clark, Clermont, Delaware, Fairfield, Franklin, Fulton, Greene, Hamilton, Hancock, Hocking, Licking, Lucas, Madison, Marion, Miami, Montgomery, Morrow, Perry, Pickaway, Seneca, Union, Warren, Wood, Allen, Defiance, Erie, Guernsey, Henry, Ottawa, Sandusky, Wyandot

How do I qualify for the grocery benefit?
The $50 monthly grocery allowance is available to MedMutual Advantage Secure HMO-POS members with qualifying chronic conditions. These conditions include cancer, diabetes, cardiovascular disorders, chronic mental health conditions and chronic heart failure. You can view a full list of qualifying conditions here.
If you are a current MedMutual Advantage Secure HMO-POS member
Complete the annual Health Risk Assessment we send in the mail. Medical Mutual will also review your claims to determine your eligibility. If you are eligible, we will mail you information about using your grocery benefit.
You should also schedule an in-home wellness visit with our trusted healthcare partner at the beginning of the plan year.
If you are a new MedMutual Advantage Secure HMO-POS member
Complete your Health Risk Assessment we send in the mail. You should schedule an in-home wellness visit with our trusted healthcare partner or have your healthcare provider submit necessary documentation confirming your condition. If you are eligible, we will mail you information about using your grocery benefit.
What can I buy with my grocery benefit?
If you qualify for the grocery benefit, you can use the $50 monthly allowance to shop for healthy foods and eat well. You can purchase groceries through the Benefits Pro portal available on your My Health Plan account or shop in person at participating retailers. You can purchase products in the following categories:
- Fresh Produce Boxes*
- Prepared Meals*
- Fresh Fruits and Vegetables
- Canned Fruits and Vegetables
- Fresh Salad Kits
- Dairy Products
- Meat and Seafood
- Beans and Legumes – peas, lentils, black-eyed peas, etc.
- Pantry Staples – flour, spices, seasonings, etc.
- Healthy Grains – cereals, pastas, etc.
- Nutritional Shakes and Bars
- Soups
- Water/Vitamin Water
Please note: You cannot use your grocery benefit to pay for non-food and pet items, alcohol, baby formula, candy, chips, coffee shop items, desserts, fresh baked goods, soda, and tobacco products.
*Fresh produce boxes and prepared meals are only available by purchasing through the Benefits Pro Portal on My Health Plan.

Grocery card is a special supplemental benefit. Grocery card is only available to qualifying members with cardiovascular disorders, cancer, diabetes, chronic and disabling mental health, and chronic heart failure (other eligible conditions not listed).
