Grocery Benefit Qualifying Conditions
In 2025, the MedMutual Advantage Secure HMO-POS plan (H6723-005-001) will include a $50 monthly grocery benefit for members in select Ohio counties who have a qualifying chronic condition. If you are a member in this plan, you may qualify for this benefit if you have a diagnosis for one or more of the conditions listed below.
Qualifying Conditions:
- Autoimmune disorders such as lupus, polymyalgia rheumatica, polymyositis and HIV/AIDS
- Cancer (Not including pre-cancer)
- Diabetes or other endocrine conditions
- Problems with your liver or kidneys such as end-stage liver disease and end-stage renal disease (ESRD)
- Heart conditions such as coronary artery disease, chronic heart failure, high cholesterol, cardiac arrhythmias and atrial fibrillation (also called A-Fib)
- Conditions that affect your lungs or breathing such as asthma, emphysema, chronic obstructive pulmonary disease (COPD), chronic bronchitis, pulmonary fibrosis and pulmonary hypertension
- Conditions that affect your mental health such as anxiety, depression, bipolar disorder, schizophrenia, eating disorders and alcohol or other drug dependence
- Brain or neurologic conditions such as Alzheimer’s disease, dementia, autism, Parkinson’s disease, epilepsy, paralysis of one or more of your limbs and multiple sclerosis (MS)
- Conditions that affect your blood vessels such as high blood pressure and history of stroke
- Severe conditions that affect your blood such as chronic anemia, hemophilia, sickle-cell disease or chronic conditions that cause blood clots

Grocery card is a special supplemental benefit. Grocery card is only available to qualifying members with cardiovascular disorders, cancer, diabetes, chronic and disabling mental health, and chronic heart failure (other eligible conditions not listed).
