Living with a chronic condition, such as diabetes, is a daily challenge for millions of people worldwide. Diabetes is a chronic condition that affects how the body processes blood sugar, leading to elevated blood sugar levels that can cause a range of health issues over time. 

People with diabetes may face physical, emotional and financial challenges as they work to keep their condition under control. Managing blood sugar levels is critical. Failing to keep them within the target range can lead to serious complications, such as heart disease, kidney damage, nerve damage and even blindness. However, balancing blood sugar levels isn't always straightforward. Factors like nutrition, exercise, stress, illness and medication can all impact blood sugar levels. 

This constant juggling act can be exhausting, as individuals must monitor their blood sugar multiple times a day, adjust their insulin dosages and keep track of what they eat and how it affects their body. Frequent medical appointments, blood sugar monitoring and maintaining a strict lifestyle can lead to burnout, making it difficult to stay motivated.

Our Chronic Condition Management Program is designed to complement the care you receive from your healthcare providers at no extra cost. This program does contain exclusions and limitations for eligibly, so be sure to check your benefits to guarantee your coverage. 

We offer a variety of digital coaching options to help you reach your health goals, such as weight loss or managing your blood sugar levels. Depending on your needs, you can choose to work with a digital coach for a few weeks, several months, or participate in quarterly check-ins. You can also sync connected devices, like a wireless glucometer, to allow your coach to help track your progress and stay motivated. 

For more personalized support, a telephonic health coach will work with you to help educate you on your condition, improve your health and manage your diabetes. After the program, you can choose to continue with your telephonic health coaching or switch to one of your digital options. 

Digital/virtual health coaching

Medical Mutual offers digital and virtual coaching options to help our members better understand their condition, prevent disease progression, provide quick updates on their progress and improve their health. You also have the flexibility to participate at times that best meet your needs. 

Digital/virtual health coaching may include:

  • Short-term health coaching delivered over a few weeks to address a specific health concern.
  • More frequent health coaching that provides personalized interactions over several months to help members lose weight, reduce blood pressure or improve bloodwork results.
  • Quarterly check-ins for members who are self-managing their care successfully.
  • Integration with connected devices, such as a wireless glucometer, blood pressure cuff or a digital scale to monitor your progress. 

Remote home monitoring 

This program leverages specialized equipment, daily digital check-ins and as-needed telephonic coaching from a nurse to help you avoid further complications and progress toward improving your health. 

No member cost sharing for diabetes testing supplies 

Many of our healthcare plans offer diabetes testing supplies with no member cost sharing if you participate in our Chronic Condition management Program. Please check your Summary Plan Description (SPD) for more information or inquire with a member of our Chronic Condition Management team. 

Telephonic health coaching 

Telephonic health coaching is designed to offer additional support in managing your chronic condition and achieving your personal health goals alongside your provider. Members in this program can work with a registered nurse, Health Coach or Certified Diabetes educator to make improvements in their health. 

Additionally, members have access to a team of professionals, including certified health educators, dietitians, pharmacists and social workers.
